* Certificate of Matriculation/ O-Level or equivalent examination
* Marks sheet / transcript of Matriculation / O-Level or equivalent examination
* Provisional Certificate (issued by the last attending high school / college) of
Intermediate (Pre-Medical) / A-Level or equivalent examination
* Marks sheet / transcript of Intermediate (Pre-Medical) / A-level or equivalent
examination
* Candidate's Domicile
* Candidates' PRC
* Candidate's NADRA National Identity Card (CNIC) / B Form
* 10 recent passport size photographs (03 photos with names attested on front and 03 on
back)
* CNIC of Father and Mother
* Domicile of Father / Guardian
